Community Living Group Home

Autumn Pathways' Community Development Services help clients build and maintain skills linked to community membership by involving them in community-based activities with people without impairments. Our community development services provide a range of community contexts and events that foster chances for more autonomy and inclusion. According to the individual’s preferences, every opportunity is explored through the person-centered planning process to support their intended results and objectives.

Respite Care Services

The goal of the Autumn Pathways Respite Care program is to temporarily relieve family caregivers of the stress and tensions of providing care, allowing them to take a break and rejuvenate. Respite care is given as needed—during an emergency or at prearranged intervals. Respite care is offered around-the-clock, every day of the week, as long as the team agrees on the authorized hours recorded in the person-centered plan. Clients in the Autumn Pathways model gain new experiences, possibilities, and self-determination that are advantageous to both them and their caregivers.

Personal Support Services

With uniquely created services, Autumn Pathway personal support services enable people with impairments live freely and participate in their community by integrating them into the community.The client receives different levels of support that improve their chances of exercising choice and control over their own lives. In collaboration, we fulfill our shared goal of making sure clients are completely integrated into their communities, workplaces, and places of worship.

Adult Residential Services

Clients can benefit from options for residential care in a cozy, secure, and well-maintained houses. Houses are occupied by no more than four people, which is licensed by the Office of Health Care Quality (OHCQ) under the communal living group home model.The residences are in the DDA Central Maryland Region and are either owned or leased by Autumn Pathways.

Day Habilitation

The goal of Autumn Pathways’ day rehabilitation program is to increase the client’s independence in a variety of life skills areas. Using formal teaching methods and engagement in activities, clients develop and maintain skills related to career and socialization, as well as instrumental activities of daily living from Autumn Pathways’ day rehabilitation services.

Nursing Support

Clients will receive high-quality nursing support services from Autumn Pathways according to their needs. Each client will receive preauthorization from DDA to receive treatment, and their person-centered plan (PCP) will also maintain this authorization. Additionally, staff supervision, training, and education regarding all nurse assessments performed on the client will be included in nursing support services.

Employment Discovery

Clients benefit from a range from flexible solutions in career discovery and employment interests with the help of Autumn Pathways employment service professionals. Our employment service professionals are available to assist clients during the day. We also support volunteer opportunities, which supports many communal settings where services can be offered.

Supportive Living

Clients in Autumn Pathways Supported Living will receive a range of customized services to help them live freely in the community. Qualified Direct Support Professionals (DSPs) assure customized, required Supported Living services tailored to the client’s interests according to the client’s person-centered plan.

Transportation

When needed, Autumn Pathways offers transportation to and from community-based events like shopping or church, as well as to and from personal and medical appointments. The advantage of clients using Autumn Pathways Developmental and Disability Services transportation: Orientation services on how to walk safely between locations by employing additional senses or aids. Obtaining volunteer transportation and mobility services, including resource access and transportation coordination.
